In episode 21 of "Good Will Society," Liu Ziyi wants to use a physical therapy device for a patient, and Gu Shiyi, understanding her intentions, goes to a doctor friend for help. Unfortunately, the doctor is off-duty.Using her social skills, Gu Shiyi manages to get the device, but only for fifteen minutes due to other patients' appointments.Because fifteen minutes isn't enough time, Liu Ziyi asks Ding Hao to stall the next patient. Ding Hao's attempt fails, and the waiting patient complains, leading to an investigation by the medical affairs department. Liu Ziyi and Gu Shiyi are ordered to write self-criticism reports.

Meanwhile, Wang Gaosheng, needing to pay off debts, plans to take on private work from another hospital. He contacts an old acquaintance to collaborate. A former patient's family, whose mother was unaware that her son had previously switched doctors without her consent, seeks out Wang Gaosheng again. The elderly woman and the patient prefer Wang Gaosheng, finding Doctor Zheng too impersonal, and they decide to switch back to Wang Gaosheng for the surgery.

In an effort to save Yang Guowei, Liu Ziyi plans to ask Huang Tianming for help. She drafts a message to Huang Tianming and coincidentally runs into Qin Wenbin, who helps her refine the wording before she sends it. Huang Tianming immediately calls Liu Ziyi back and establishes a foundation, ensuring that Yang Guowei's treatment can continue. However, Liu Ziyi and the head of the medical affairs department are reprimanded by the hospital director for going over his head. Their actions put the director in a difficult position, as the hospital had its own plans.

Yang Guowei's wife returns the clothes she had borrowed from Liu Ziyi, bringing fruits from their hometown as a thank you. She tells Liu Ziyi that they had planned to return to their hometown at the end of the month to manage a plot of land they had contracted, hoping for a peaceful life away from the city. However, tragedy strikes. Yang Guowei has a medical emergency. Liu Ziyi rushes to the ward to attempt a rescue, but it is unsuccessful. Yang Guowei's wife is devastated, her cries echoing through the quiet corridor, and Liu Ziyi is deeply affected by the loss.

Qin Wenbin sees Liu Ziyi's distress in the parking lot and gets in her car to talk. However, Qin Wenbin can't stay long, as he has a dinner appointment with Wang Gaosheng and Doctor Zhang. At the dinner, Qin Wenbin overhears their conversation and suspects Wang Gaosheng is planning to take on private cases. He warns Wang Gaosheng about the risks, including potential dismissal or revocation of his medical license, and the possibility of imprisonment if a medical accident occurs. Wang Gaosheng lies to Qin Wenbin, claiming he's just meeting with Doctor Zhang and has no intention of taking on outside work.

A security guard, Xiao Hu, performs CPR on a girl who fainted on the bus to work. Despite being a security guard, Xiao Hu has been diligently studying and is knowledgeable about first aid, earning praise from Gu Shiyi for performing CPR during the crucial first four minutes. However, the security captain believes Xiao Hu has caused trouble for the hospital, fearing the patient might not pay her medical bills. As predicted, the rescued patient remains silent and refuses to disclose her name to the nurses after regaining consciousness. Wang Gaosheng's mother visits the hospital and overhears the nurses gossiping, realizing her son is also struggling.
